If you have an existing patient portal account, you may login and access your medical records. For a copy of your complete patient records please complete the records release form in your patient portal. Send a secure message with any questions.
If you are a previous patient of Focus Health Group but do not have a current or prior patient portal account, you may gain access to your patient portal records by creating an account. Please click here to create your account.
Download and submit a records request form to obtain a copy of your records for yourself or to have them shared with a new provider. Click here to download the form. Once downloaded you may print and mail to 448 Ignacio Blvd #294, Novato CA 94949, or by Fax to 866-484-0518, or send through the patient portal.
